We believe that all accidents occur at the intersection of three interacting elements: the Human, the Machine, and the Environment in which a particular event occurs. Environment includes both social and environmental factors. Traditional accident investigations focus almost exclusively on the Machine element of the accident–our focus is on a thorough understanding of all three factors and how each influences the others. Only by examining all three factors and the interrelations between them can a true picture of the occurrence be obtained.
